What do I need to bring along to my first appointment?It is helpful if you have any relevant information or reports from your General Practitioner (GP) or a discharge summary report from a recent hospital admission. If you are a NDIS participant, a copy of your NDIS Plan Goals is also helpful to guide our assessment and identify what areas you may require our assistance.

How do I complete a telehealth appointment?Prior to commencement of Telehealth services, we will assist you with set-up through provision of easy-to-follow instructions. We provide an appointment time and link to join the session via email to allow ease of access to services.

Can I use my NDIS funding?We support clients who are self managed and plan managed.

What steps do I need to take in order to have a driving assessment?Book an appointment with your GP and advise them you wish to obtain a medical certificate for motor vehicle drivers (form F3712 in Queensland) Provide this medical certificate and complete the relevant documentation (form F4355 in Queensland) to notify the Department of Transport and Main Roads of your condition, to update your existing licence or apply for a learner’s permit Complete any visual assessments by an Optometrist or Ophthalmologist if this was recommended by your GP. Book an Off-Road Assessment with your Driver-Trained OT. Once this has been successfully completed, the OT will organise an On Road Assessment with a recommended driving school, including the use of a specialised vehicle if required (depending on whether you need to use specialised driving controls). Complete the required number of lessons and hours in your graded driving rehabilitation program, which can be determined by the type of modifications or driving controls recommended and driving experience. Your OT will assist you to complete a report for the NDIS, for funding for modifications and driving controls if you are NDIS funded.
